SERVICE COORDINATION

The Service Coordinator assists the person in coordinating and monitoring all services that the person receives. Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant’s independence, productivity and integration into the community while maintaining health and welfare of the individual. The Service Coordinator is responsible for assisting the participant, who is the primary decision maker, work together to develop and implement a plan that meets the participant’s goals. The Service Coordinator is an effective advocate for the participant to ensure the participant is receiving appropriate and quality services from all providers.

HOME HEALTH SERVICES

Hickok Home Care Services provides 24 hour care 7 days per week. Our qualified and dedicated health care professionals will be there when you need them the most. Their Dependability and compassion to your needs is second to none. Regardless of your need and situation, Hickok Home Care will be there for you. That is our commitment to you.

Hickok Center for Brain InjuryDAY PROGRAMS

The Hickok Center’s structured day program/day habilitation is based on
a consumer driven model. All program participants take an active role in the operation of the organization. This includes planning the monthly activity calendar, running groups, performing clerical functions, performing receptionist functions, fundraising activities, all which provide pre-vocational and social/recreational training and support. Members have the opportunity to practice and re-develop the skills they need to succeed in vocational training, community interaction, and personal relationships. Hickok Center for Brain InjuryINDEPENDENT LIVING SKILLS PROGRAM (ILST)

Independent Living Skills Training is an individualized service designed to improve the ability of the participant to live as independently as possible in the community. Services may include assessment, teaching, and training relating to self-care, medication management, task completion, communication skills, interpersonal skills, socialization, sensory/motor skills, mobility, community transportation, reduction or elimination of maladaptive behaviors, problem solving skills, money management, pre-vocational skills and skills to maintain a household. read more

COMMUNITY INTEGRATION COUNSELING(CIC)

Community Integration Counseling is an individualized service designed to assist the participant to more effectively manage the emotional difficulties associated with the adjusting to and living in the community. It is a counseling service provided to a participant coping with the altered abilities and skills, the need to revise long term expectations, and changed roles in relation to significant others. This service is available to participants and/or anyone involved in an ongoing significant relationship with the participant when the topics are directly related to the participant.


VOCATIONAL SERVICES

The Hickok Center for Brain Injury provides vocational services that are referred by and funded through ACCES-VR. These services include job exploration, job placement, job coaching and extended services to ensure success on the job.
